Muscle or Joint Problems? – Self-refer to Physio Line

You can now self–refer to the Physio Line telephone service for muscle and joint problems if you meet the following criteria:

• You are aged 18 or over
• You have had your condition for less than six months
• Your complaint is regarding a single (1) joint/area

You can complete the self-referral form electronically and send it by email to msk.bedfordshire@circlehealthgroup.co.uk

Alternatively, you can print a copy of the self referral form and send it by post to:

If you would like to self-refer to Physio Line but are unable to access the self-referral form. Please ask reception for a copy.
