Using an online form

There are many things you can do online at our surgery. Some tasks you can do using a website called Klinik where you can:

  • request an appointment
  • request a sick note
  • ask about test results
  • contact us about an admin request

Using your NHS account or SystmOnline

There are other tasks you can do online, like:

  • order repeat prescriptions
  • view your test results
  • access your health record
  • check or cancel appointments
  • see your vaccination record

For these, you can use your NHS account (through the NHS website or NHS App) or SystmOnline.

Setting up an NHS account

To set up your NHS account for the first time, you will need to set up an NHS login. Read about how to set up NHS login.

Once you have set up your NHS login, you can log in to your NHS account or download the NHS App onto your phone.

Setting up SystmOnline

To set up SystmOnline for the first time:

  • visit the surgery with photo ID so we can confirm your identity
  • we will give you a username and password
  • go to SystmOnline or use the app to register

If you forget your password, visit the practice with your photo identification, and we will issue you a new password.

Frequently asked questions

I can’t request my medication

If you can’t see the tick box to request your medication, here are a few possible reasons:

  • It might be too early to request your medication. You can only ask for a further supply seven days before you run out.
  • You may need a medication review. 
  • Your medication might require special approval from a doctor.
  • The medication might not be on your list of repeat prescriptions.
  • If you’re a new patient, you might not be fully registered with the practice yet.

You can still request your medication by writing it down in the custom request box at the bottom of the ‘request medication’ page within SystmOnline.

If you normally request medication with the NHS app, use our KLINIK platform to submit a repeat prescription request.

I am due for a medication review

We are keen to ensure that patients with ongoing medical problems are monitored regularly.

If you have been advised by the surgery that your medication review is due, please use our online Medication Review form or inform reception. This will allow the dispensary to arrange for a pharmacist to subsequently carry out a medication review – they may not need to call you to undertake this.

We will not leave you without your medication unless the clinical risk outweighs the benefit.

If you are unable to order medications online because of an overdue medication review, you can request your medications through our online consultation service (Klinik). If you are using SystmOnline, you can submit a custom request. This is located beneath the tick boxes where you would normally request your medication.
