Register with the surgery

Catchment area

Before filling out a registration form, check you are in our catchment area.

You can also find out which GPs you can register with on the NHS website.

To complete your registration, you must provide proof of identification for safeguarding purposes.

Already registered with the NHS app? Logging in before completing the registration form will allow you to register with our practice without submitting a photo and address ID.

It may take up to 1 to 2 weeks to process new registrations.

If you’re running low on your medication, be sure to contact your current GP for another supply before registering with our practice. If your previous practice has deregistered you, you can contact NHS 111 for urgent supply.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

If you are unable to register online, you can either come into reception and pick up a paper form or you can print out the form below and bring it into reception along with your photographic ID.
